This application is meant for Physicians and intensivists who confront patients with renal failure and are on antimicrobials. The dosage adjustments are based on the guidelines by the American college of Physicians.
The data displayed are
GFR value based on cockroft-gault formula
Dosage required for a particular drug
Dosage adjustment over the full range of GFR
Dosage adjustment for Intermittent hemodialysis, if any.
This excellent review includes guidelines for over 130 drugs including anti-bacterials, anti-virals, anti-fungals and anti-parasitic agents.

I´ve downloaded it a few mouths ago, it was a good app untill I changed my IOS (I stil have IPAD1) to the version 4.3. Then it has stopped functioning. So I did not like it any more.

Inadequate
Does not calculate appropriately the once-daily dosage of gentamicin, compared to the "Sanford" I will delete this calculator from my Ipod

Dosing App Doesnt Work
Bug in antibiotic selection - even when drug entered into program gives "info-please select antibiotic" message. Never provides a calculation. Of note, not sure how the program would have calculated renal function per Cockcroft-Gault anyway given that it doesnt ask for patient height.
